What's the max I can expect from an 8320 with push-pull hyper 212 evo, 5 120mm case fans? A friend said it should do 4.5.

With my 8350 and 212 evo I can get 4.4GHz fine but 4.5GHz gets a bit toasty (65C prime95 socket temp). It's okay with things like BF4 though, keeping at a nice 50-55C at the socket. As far as I know the recommended max temp for the socket is about 62C. It mostly comes down to the voltage your processor requires at them clocks. In contrast the same cooler handled my 3570K at 4.6GHz quite well.

When I added a second fan to do push-pull I only noticed about 2C lower temps.

95w tdp vs 125tdp. 30watts of heat makes a difference on a small cooler.
Pretty much this.

I have a 2600k on an H-60 that can prime at 5 ghz and stay reasonably cool, but the H-60 wouldn't have a chance over 4.5 ghz or so on an 8 core Vishera.
